Medication Permission Form
Physical Examination Form
There have been several changes in the forms that we are using and the archdiocesan policies
concerning health and medications.
- All “ new to the school” students need to have the physical form filled out by a physician.
- Also you must have an up to date immunization form at the school before school begins or on the first day of school. The immunization requirements have changed for kindergarten, sixth graders and new students, so please check with your doctor concerning your child’s immunizations.
You will be receiving a form from the teachers during the first week of school. This is different from the clinic form and will be kept by the teachers in their classrooms. These will be used for field trips and to keep the teacher up to date with their students. They must be returned the first week of school to the homeroom teacher.
- If your child has a medical condition that require treatments , medications on a regular or semi regular basis, life threatening allergies, asthma, inhalers, etc., please contact the school nurse, Patty Finley, either by email pfinley@stmga.org or by phone 404.373.2586 ext.129, to work out a plan of action for your child
- Over the counter medications that are given on an as needed basis, such as Advil or Tylenol for headaches, new braces etc., must have a medication form signed by a doctor and the bottle must be labeled with the child’s name and dosage. These forms are available in the office or on the STM web site. This is very different from last year when parents could send in medications and they could be given with just the parent’s permission. The form is good for one school calendar year, and must be renewed every year. Each child needs a separate form if they have a medication. One form will not cover several family members. Please note that a stamped doctor’s signature will not be accepted only a hand written signature will be accepted.
- If your child has antibiotics that need to be taken during school, you must have a doctors order that it be given.
- If there is ever a change in your child’s medical condition or medications are started or stopped, please notify us so we can continue to watch out for your child’s safety and well being.
